Title VI Program:  Complete a 506 form and provide Tribal Nation Documentation of the qualifying student, parent, or grandparent.

JOM Program:  Complete a 506 form AND provide the STUDENT’S tribal membership card or Tribal Nation documentation for the STUDENT.
